Transfer stuck on 'pending losing registrar' for over a week

Started a .com transfer 9 days ago, approved the registry email straight away, but the panel still says 'pending losing registrar approval'. My old registrar swears they've done nothing to block it. Shouldn't this auto-approve after 5 days? Feels like the request never actually reached them.
