Sender rules hit_count never increments
allow/block rules show 0 hits forever, even though scanner.log clearly shows them firing. Tested with a swaks message matching the gmail.com BLOCK rule and counter stays at 0.

Scanner was never UPDATEing sender_rules — only logging the match. Added _bump_sender_rule_hit() that increments hit_count and updates last_hit_at on match. Permissions were already in place.
