Postscreen rejecting legitimate Gmail/Outlook on first connection
Postscreen's mandatory deep-protocol test was returning 450 4.3.2 to Gmail and Outlook on first attempt. Sender servers retry and it works the second time, but we're losing some first-attempt mail entirely.

Bypassed postscreen entirely — master.cf now goes smtp inet → smtpd directly. The DNSBL scoring still works via the body content scoring path, so we don't lose much. Trade is more scanner CPU but zero legit-mail loss.
